Blueprints for the Community Grant

Ingleside Homes is a nonprofit organization providing affordable housing, healthcare, and supportive social services to low-income seniors in Wilmington, Delaware. Ingleside's program increases access to healthcare while addressing social determinants of health equity, such as food security, housing stability, under-insured individuals, educational barriers, ethnic and medical language barriers, and social and community support.

Highmark Blue Cross Blue Shield Delaware's BluePrints for the Community (BluePrints) Grant Program began in 2007 to support communities and organizations in Delaware. This grant program funds health-related projects and programs that address the BluePrints for the Community priority areas to improve health outcomes in Delaware.

IHI has decades of experience and data on the elderly population served in Delaware. IHI's subsidized programs provide equal access to the same health services to all seniors, regardless of race, gender, income, or marital status.

Both organizations have focused on solving the issue of health disparities for the frail and low-income seniors in Delaware. They recently worked together to negotiate and adjust the funding reimbursement rates to meet the cost of supplying healthcare services.

The grant will provide 45 low-income seniors access to housing, social services, meals, and transportation to stores for one month.
