Connecting Those Aging-at-Home Reduces Hospital Re-Admissions and Helps Spread the Burden of Care
MyHomeReach is a tablet based unified communications service that connects care-recipients, care-givers, family members and local service organizations via a simple to use 'one-touch' tablet-based system. Care givers are then able to monitor the health status and quality of life conditions for seniors and/or those managing chronic health conditions. Care recipients are able to communicate their health status via simple to use large on screen 'buttons' that convey their status, monitor their health vitals and guide them through a health conditions dialogue that further defines their current conditions and aids in determining if a pro-active care giver intervention is required.
MyHomeVision is an in-home tele-presence video console system that allows remote care givers and family member to engage in an immersive video tele-presence session including, if permissible, to remotely control the view the tele-presence system has to survey the status, surroundings and living conditions of in home care recipients.

Also available for demonstration will be the company's tablet based point of care forms data capture and submission system.
