ChartaCares to Launch Its Community Health Connection Services MyHomeReach and MyHomeVision
Connecting Those Aging-at-Home Reduces Hospital Re-Admissions and Helps Spread the Burden of Care
MyHomeReach is a tablet based unified communications service that connects care-recipients, care-givers, family members and local service organizations via a simple to use 'one-touch' tablet-based system. Care givers are then able to monitor the health status and quality of life conditions for seniors and/or those managing chronic health conditions. Care recipients are able to communicate their health status via simple to use large on screen 'buttons' that convey their status, monitor their health vitals and guide them through a health conditions dialogue that further defines their current conditions and aids in determining if a pro-active care giver intervention is required.
MyHomeVision is an in-home tele-presence video console system that allows remote care givers and family member to engage in an immersive video tele-presence session including, if permissible, to remotely control the view the tele-presence system has to survey the status, surroundings and living conditions of in home care recipients.
"We believe that this conference provides a premier opportunity for those guiding the formulation of state and municipality led care practice policies to experience firsthand the significant advances in technology enabled health care for the growing population of seniors aging at home in their communities. Market research has shown that the rates of hospital re-admissions and in congestive heart failure can be reduced with applications that connect care recipients in a timely and easy to use manner," said Lauryn Schimmel, managing director of ChartaCloud.
The company also stated that it will have on hand and will demonstrate its humanoid robot, ZORA, being used in assisted living facilities to augment care giver activities such as exercise classes, rehabilitation therapies, community social engagement activities and specialized therapy sessions for those suffering cognitive impairment. Also available for demonstration will be the company's tablet based point of care forms data capture and submission system.
"The burden on communities is rapidly growing and we believe that the use of our applications will help enhance care, reduce the burden on first responders and hospitals and will enhance the roles played by local community service organizations serving seniors and the family members of age-at-home seniors. We are excited to establish working relationships with local communities,"
