UCLA Health Systems Data Breach Class Action Investigation

The breach may have begun as early as September 2014, and breached data may include the following information: patient names, dates of birth, Social Security numbers, Medicare and health plan identification numbers as well as some medical information such as patient diagnoses and procedures.
