Thanks to a groundbreaking project called the Comprehensive Primary Care (CPC) initiative, developed by the Centers for Medicare & Medicaid Services (CMS) Innovation Center. CMS is collaborating with a select group of health plans – including Hudson Health Plan – and physician groups to demonstrate that paying primary care professionals to coordinate care of patients with complex medical needs can improve patient health and reduce overall health costs. Today, Georganne Chapin, President & CEO of Hudson, announced that 13 Hudson providers have been selected to participate in the CPC initiative. “For this important initiative, CMS picked forward-thinking physician groups that have demonstrated their commitment to delivering high quality care to patients in the Hudson Valley Region and Capital District. The fact that 13 of the groups are Hudson providers indicates the high quality of care we offer our members,” Ms. Chapin pointed out. Hudson will begin paying a care coordination fee to the following Hudson providers that CMS has selected to participate in the CPC initiative: Dutchess County Fishkill Medical and Associates (Fishkill) Health Quest Medical Practice Primary Care (Hopewell Junction, Hyde Park, Lagrangeville, Millbrook, Poughkeepsie) Hudson Valley Primary Care (Wappingers Falls) Village Medical PLLC (Pawling) Orange County New Windsor Family Medicine (New Windsor) Putnam County Northern Heart Specialists (Carmel, Mahopac) The Westchester Medical Practice P.C, (Cold Spring) Rockland County Rockland Family Medical Care (West Haverstraw) Sullivan County River Valley Family Medical Services P.C. (Barryville) Middletown Medical, P.C. (Ferndale) Ulster County Eugene P. Heslin, M.D., PC (Saugerties) Health Quest Medical Practice Primary Care (Highland) Westchester County Comprehensive Primary Care Services (New Rochelle) The Westchester Medical Practice PC (Yorktown Heights, Peekskill) Westchester Health Associates (Thornwood) In addition to compensating participating providers for coordinating care, Hudson will provide valuable data to identify patients in need of closer supervision and to alert physicians when patients visit specialists and hospital emergency departments or are released from a hospital. Both CMS, as a payer for Medicare services, and private insurers will pay primary care practices a care management fee to support enhanced, coordinated services. These investments in primary care practices will allow doctors to spend more time with their patients, expand office hours, provide additional services such as nutrition or smoking-cessation counseling, and coordinate care for their patients. CMS has selected seven regions in the nation to participate in the CPC initiative: Arkansas, Colorado, New Jersey, Oregon, New York’s Capital District-Hudson Valley Region, Ohio’s Cincinnati-Dayton Region, and greater Tulsa in Oklahoma.
