Family Health Team Delivers Test Results To Patients Using AnyWare Group’s Patient Portal

TORONTO – March 9, 2011 - Any Ware Group, a leading supplier of healthcare portals, today announced that the West Carlton Family Health Team located in Ottawa, Ontario is using their patient portal technology to provide patients secure online access to their test results. AnyWare Group’s patient portal has been integrated into West Carlton’s Clinical Information System (electronic medical record or EMR) allowing patients to view their test results, update their demographic information and access educational materials. AnyWare Group’s patient portal was implemented to improve a patient’s experience with regard to self management of their health and reduce the administrative costs associated with phone calls from patients requesting information, such as recent laboratory results.

For our patients it means having the convenience of knowing their results without repetitive calls to our office and having to maintain a record of their own. The portal also has tools for patients and providers to establish self management process around various lifestyle risk factors. Patients can access education on risk factors, including the associated health risks and life style changes they can implement in their daily lives to reduce these risks. Patients can also set a personal goal for improving a risk factor and track their progress in the portal. Unique to this implementation is the ability of the patient to add their own data such as blood pressure, weight and waist measurements, which is integrated into their medical records allowing the health care team to monitor any significant changes.

The Family Health practice has been in existence in Carp since 1973. It started with just two physicians and currently has ten physicians, Nurse Practitioners, Dietitian, Pharmacist, Social Worker and other patient support staff.
