Murphy, South Nassau’s president and CEO. “Fulfilling this commitment ensures that the patients and communities we serve have access to high-quality, patient-centered healthcare services that they deserve.” South Nassau is one of 1,043 hospitals in the U.S. to earn the distinction of The Joint Commission has named South Nassau Communities Hospital a Top Performer on Key Quality Measures® for the third consecutive year. South Nassau earned the award for its exemplary use of evidence-based clinical processes that are shown to improve care for the following conditions and services: heart attack; heart failure; pneumonia; surgery; children's asthma; inpatient psychiatric care; venous thromboembolism; “Our organization is steadfast in its commitment to achieve the highest standards in patient safety and quality,” said Richard J. The ratings are based on an aggregation of accountability measures data reported to The Joint Commission during the 2014 calendar year. The list of Top Performer organizations represents 31.5 percent of all Joint Commission-accredited hospitals reporting accountability measure performance data for 2014. The Joint Commission Top Performer honor joins a multitude of others South Nassau has earned for outstanding quality in 2015, including: ü ‘A’ for patient safety fromThe Leapfrog Group; ü American College of Surgeons Commission on Cancer Outstanding Achievement Award; ü Home Health Quality Improvement Gold Medal ü Named to Home Care Elite list 10th consecutive year; ü New York State Department of Health’s Perinatal Quality Collaborative Obstetrical Improvement Quality Improvement Award; ü American Heart Association/ ü American Heart Association/ To earnTop Performer on Key Quality Measures, South Nassau: · achieved a cumulative performance of 95 percent or above across all reported accountability measures; · achieved a performance of 95 percent or above on each and every on each and every qualifying accountability measure reported; · and had at least one core measure set that was a composite rate of 95 percent or above, and (within that measure set) all applicable individual accountability measures had to have a performance rate of 95 percent or above. Each accountability measure represents an evidence-based practice – for example, giving aspirin at arrival for heart attack patients, giving antibiotics one hour before surgery, and providing a home management plan of care for children with asthma.
