SYRACUSE, N.Y. - March 17, 2014 - PRLog -- Breast Care Partners, LLC, (BCP) is a unique partnership between dedicated breast care specialists from CNY Surgical Physicians, St. Joseph’s Physicians Surgical Services, Hematology Oncology Associates of CNY, Crouse Radiology Associates and Crouse Hospital. This partnership was made to streamline the management of malignant and benign breast disease.
Program Director Pat Donnelly and Breast Cancer Nurse Navigators Kathleen Tindall, RN, and Emeline McShane, RN, led the implementation of Oncology OnTrack. They worked with the Priority Consult team to identify the ways that Oncology OnTrack could help them provide quality care more efficiently. The specialized care they provide involves assigning a certified Nurse Navigator to guide each patient through individualized evaluation and course of treatment. Their multidisciplinary team meets weekly to discuss patient cases and develop treatment recommendations. BCP is pursuing national certification and is also utilizing Oncology OnTrack to help track and analyze de-identified patient data that will be used for quality improvement and benchmarking their program with other centers in the country. Priority Consult, LLC, provides technologically advanced workflow solutions for the care of spine and oncology patients. The results of these programs are improved patient care by streamlining processes, expedited communication between clinicians and patients, and standardized collection of clinical data.
