The NVNA Works Transition Program offers a health aide to help guide and ease the transition of moving from one’s home to assisted living or a nursing home. “The aide can spend time assisting the person in their home prior to the move. This develops a familiarity between the person and their caregiver” says Christine McDonough, NVNA Works Director. “When the move happens, the client feels the comfort of having someone who they are familiar with right alongside to help introduce them to their new surroundings, reassure them, as well as assist and advocate for them as needed.” McDonough points out that “having the one on one aide by their side to help get them through the first night or two as well as helping with the initial adjustment to their new home can go a long way in helping someone to feel comfortable with their new surroundings.
