High Risk Renal Cell Carcinoma Surgery Successfully Performed at Moolchand

A rare surgery on a 23 year old Nigerian girl Akani Chimenem Paul suffering from kidney tumour (Renal Cell Carcinoma) was successfully conducted at Moolchand Medcity.

A rare surgery on a 23 year old Nigerian girl Akani Chimenem Paul suffering from kidney tumour (Renal Cell Carcinoma) was successfully conducted at Moolchand Medcity.  A team of eminent super-specialists from Moolchand took up the challenge and performed the surgery successfully by removing the 4 kg tumour with minimal blood loss.  The team of Moolchand physicians who operated this rare case included Dr. Shanti Vardhan, Dr. S V Kotwal and Dr. Ashwin Garg.

The patient had pain in abdomen since 2006 with other symptoms such as loss of weight and appetite, generalized body ache, post-prandial abdomen distension and bowel habits.  In November 2008, a surgery was attempted on her in Nigeria but was declared inoperable after opening the abdomen because the tumor diagnosed was very vascular.  Thus, the operating surgeon decided not to go ahead because of the high risk of the surgery and the patient was further advised for chemotherapy in Nigeria.  She was clinically diagnosed as Renal Cell Carcinoma which is notorious for excessive bleeding and every surgeon thinks twice before operating any such case.  

In April 2009, the patient and her family decided to visit India for second opinion and chose Moolchand Medcity for their healthcare needs.  “We came to Moolchand with a hope of saving our daughter’s life.  The moment we arrived here, we felt completely safe in the hands of expert doctors.  Considering the high risk case of our daughter, we are extremely content on seeing her in good health now”, said Mrs. Stella Ichenwo, mother of the patient.

Dr. Shantivardhan, Senior Consultant, Oncosurgery, Moolchand Medcity said, “The patient’s condition was very critical as with time (3 years), the size of tumour had increased (with normal size of the kidney i.e. 300 gm to approximately 4 kg) and was occupying half of the abdomen.  Our team at Moolchand clinically diagnosed the patient for renal cell carcinoma, i.e. kidney cancer and we suggested the patient to undergo tumor removal surgery before starting chemotherapy.”

Dr. S V Kotwal, Consultant, Urosurgery, Moolchand Medcity removed the kidney tumour of 4 kg from her body.  He said, “This was a very large kidney tumour invading the pancreas and a part of the pancreas too had to be removed along with it.  The surgery was particularly difficult due to dense adhesions following the previous failed operation at Nigeria.  Fortunately, since angio-embolisation had been performed pre-operatively, we were able to remove the tumour safely with only four units of blood transfusion.  Her post surgery recovery is progressing well according to the norms and expectations.”

“Renal cell cancer, which has the propensity for rapid enlargement and local invasion, may present a serious challenge to surgeons, in part because of extensive blood supply,” says Dr. Ashwin Garg, Consultant, Centre for Interventional Radiology, Moolchand Medcity.  Elaborating further he said, “In this case, pre-operative renal artery embolisation (occlusion) blocked at least 80% of blood flow to the tumour with no peri-procedure complication.  This helped the surgeons to remove the cancer with decreased blood loss.  Also, other benefits of this procedure are technical facilitation of tumor removal, cutting down the time of surgery, and danger of venous displacement of tumor-cells. Moreover, it is associated with reduced hospital costs, due in part to decreased hospital length of stay.”
