Rinvoq (Upadacitinib) belongs to a group of medicines called Janus Kinase (JAK) inhibitors. The JAK family includes four proteins: JAK1, JAK2, JAK3, and TYK2, which help immune cells respond to signals from cytokines, the proteins that regulate inflammation. Normally, these signals activate the JAK-STAT pathway and switch on genes that drive inflammatory responses. Rinvoq selectively blocks JAK1, reducing the activation of STAT proteins and lowering the expression of inflammation-related genes. As a result, cytokine signaling becomes less intense, the immune response is better controlled, and inflammation-related symptoms can be reduced.

Ideal candidates eligible for Rinvoq

1.  Adults with moderately to severely active rheumatoid arthritis who have had an inadequate response or intolerance to one or more TNF blockers.
Note:  Rinvoq is not recommended for use in combination with other JAK inhibitors, biologic DMARDs, or with potent immunosuppressants such as azathioprine and cyclosporine.

2.  Adults with active psoriatic arthritis who have had an inadequate response or intolerance to one or more TNF blockers.

3.  Adults and pediatric patients 12 years of age and older with refractory, moderate to severe atopic dermatitis whose disease is not adequately controlled with other systemic drug products, including biologics, or when use of those therapies are inadvisable.

4.  Adults with moderately to severely active ulcerative colitis who have had an inadequate response or intolerance to one or more TNF blockers.
Note: Rinvoq is not recommended for use in combination with other JAK inhibitors, biological therapies for ulcerative colitis, or with potent immunosuppressants such as azathioprine and cyclosporine.

5.  Adults with moderately to severely active Crohn's disease who have had an inadequate response or intolerance to one or more TNF blockers.
Note:  Rinvoq is not recommended for use in combination with other JAK inhibitors, biological therapies for Crohn's disease, or with potent immunosuppressants such as azathioprine and cyclosporine.

6.  Adults with active ankylosing spondylitis who have had an inadequate response or intolerance to one or more TNF blockers.
Note:  Rinvoq is not recommended for use in combination with other JAK inhibitors, biologic DMARDs, or with potent immunosuppressants such as azathioprine and cyclosporine.

7.  Adults with active non-radiographic axial spondyloarthritis with objective signs of inflammation who have had an inadequate response or intolerance to TNF blocker therapy.

In special groups:
1)  Lactation: It is advised not to breastfeed while taking Rinvoq
