Through this expansion, the organization strives to address the evolving healthcare needs of the community by offering a wide range of services, including adult primary care, chronic disease management, on-site laboratory, behavioral health, and medical case management.
FoundCare CEO Christopher Irizarry expressed enthusiasm about the expansion and its potential impact on the community. The expanded adult primary care services will be delivered by a team of highly skilled healthcare providers, including physicians, nurse practitioners, and support staff, all dedicated to delivering personalized and compassionate care. Services will be available Monday through Friday from 8 a.m. to 5 p.m.

FoundCare's mission is to provide quality healthcare and social services for all individuals and families. Currently, FoundCare has seven locations throughout Palm Beach County spanning from North Palm Beach to Boynton Beach and accepts most insurance plans, Medicaid, Medicare and self-pay on a sliding fee scale based on household size and income. No one is turned away due to inability to pay.
