TALLAHASSEE, Fla. - Jan. 5, 2023 - PRLog -- Chronic care management (CCM) is a preventative service defined by the Centers for Medicare and Medicaid Services (CMS). CCM is generally non-face-to-face and provided to Medicare beneficiaries. Chronic conditions are expected to last up to 12 months or the death of the patient. The main goal of chronic care management is to assist patients while reaching better life quality through their conditions.
