NASHVILLE, Tenn. - July 13, 2021 - PRLog -- REACH Kidney Care announced today that it received three-year Population Health Program (PHP) accreditation from The National Committee for Quality Assurance (NCQA). NCQA accreditation is awarded to organizations meeting stringent quality standards and is intended to encourage a culture of continuous improvement. Achieving NCQA accreditation is reflective of REACH's commitment to transforming care for individuals with kidney disease to ensure the highest quality of care and quality of life possible.

"We are honored to receive NCQA Population Health Program Accreditation. This accreditation is a milestone achievement for REACH and illustrates our dedication to providing the best care to individuals with kidney disease," said Doug Johnson, M.D., Co-founder of REACH Kidney Care and Vice Chairman of the Board for Dialysis Clinic, Inc. "Working in collaboration with patients, their families and their other care team members, REACH is able to improve kidney health outcomes and address the population's complex needs."

Using a population-based approach to ensure efficient, high quality processes are in place allows the patient to remain at the center of their care. This approach helps keep members healthier, reduce risks and prevent unnecessary costs.

"The Population Health Management programs moves us in greater alignment with the increased focus on person- centered population health management," said Margaret E. O'Kane, President, NCQA. "Not only does it add value to existing quality improvement efforts; it also demonstrates an organization's highest level of commitment to improving the quality of care that meets people's needs."
