Communities Clinical Manager Announcement

INDEPENDENCE, Mo. - June 30, 2021 - PRLog -- Missouri Palliative & Hospice Care, a community-based organization, is proud to announce the promotion of Heather Sanchez, RN, BSN from RN Case Manager to Communities Clinical Manager. Sanchez has been with our organization since July 2018 and continues to excel and thrive as a member of our team. Sanchez is commended by peers, patients and families on her patient care, empathy and supportiveness.

She works well with all of our staff and the staff in the communities to make sure the needs of our patients and their families are met."

Sanchez will develop and maintain clinical relationships within her assigned communities by providing ongoing support and tailoring the plan of care to the needs of the communities' while being a resource to our clinical team. She will support the referral to admission process by completing informationals, gathering the information needed, assisting and following up on admission needs and recognizing and meeting patient and community needs. Sanchez will also establish and maintain the plan of care to ensure compliance with company and communities' policies, procedures and Centers for Medicare and Medicaid Services (CMS) regulations. Sanchez's take-charge attitude, patient advocacy, clinical skills and hospice knowledge will educate the patients, families and communities on the Medicare Benefit and the incredible care we provide to our patients and families.
