Our service includes; comprehensive assessment, advocacy, support planning, ongoing care management and review.
At Liberty, we understand that everyone's needs are different and what's right for one person isn't always the right decision for another person. That is why our consultation service is free of charge.

As an ethical company our aim is to support people's physical health and emotional wellbeing, by preventing social isolation and supporting access to health and social care services. If during the free consultation period, we are not the right people to support you, we will either signpost you to another organisation or with consent, make appropriate referrals on your behalf.

Our range of services include Mental Capacity & Best Interest Assessments, Advocacy, Support Planning, Independent Social Care Needs Assessments and Independent social worker reports.
