MUMBAI, India - Sept. 26, 2016 - PRLog -- Vacuum aspiration or suction aspiration is a surgical method of abortion. As the name suggests, vacuum or suction is used to empty the uterus of its contents. Suction force is created either by the means of using electricity or manually by using a 50 cc syringe. When electricity is used for creating vacuum, the procedure is called EVA or Electric Vacuum Aspiration and when a manual syringe is used for suction, the procedure is called Manual Vacuum Aspiration or MVA. Vacuum aspiration has become the method of choice for surgical abortion, replacing the more invasive dilatation and curettage.

Vacuum aspiration is an outpatient procedure, which does not require general anaesthesia. It is an ideal procedure during the early first trimester. As it is an outpatient procedure, there is no need for hospital admission. The procedure is done under local anaesthesia. Cervix is dilated either by use of medications or by using cervical dilators. A local anaesthetic is injected through the cervix into the uterus. Following this, a flexible cannula is inserted into the uterus. The cannula is attached to the source of vacuum. The procedure is carried out under ultrasound guidance. On applying suction, the contents of the uterus are aspirated out resulting in termination of pregnancy. After the procedure, a confirmatory ultrasonography is carried out to make sure that the uterus has been emptied of all its contents. The whole procedure takes approximately 15 minutes. Following the procedure, the woman is kept under observation and her vitals like blood pressure, pulse rate and temperature are monitored.

This procedure is one of the safest obstetric procedures and results in complete abortion in 99.5% when carried out early during the first trimester. Complication rates are lower as compared to dilation and curettage and medical abortion. The procedure is also less painful as compared to the other two methods. Since no sharp curettes are used, trauma to the uterus is significantly reduced. The procedure is also cheaper as compared to dilation and curettage. Rarely, the products of conception maybe retained partially within the uterus, in which case a second vacuum aspiration is carried out.

Common adverse effects following the procedure are nausea, cramping and abdominal pain. Rarely, more serious adverse effects may occur. These include uncontrolled haemorrhage, incomplete evacuation of the contents, uterine tear or cervical tear. In case of a cervical tear or a uterine tear, it is essential to check for internal bleeding and repair the tear. This is done by the means of laparoscopic or conventional surgery.

There is not much difference in the safety profile between manual aspiration (MVA) and electrical aspiration (EVA). However, manual aspiration is slightly cheaper as the equipment required is cheaper. Some doctors report that women prefer manual aspiration (MVA) as there are no suction instruments used and they feel less intimidated. Both the methods have proven to be highly effective and safe in early first trimester abortions.
