By: Consortium Media Caregivers are now able to access the current Health Education Passport (HEP) in the electronic format by logging onto the website from a computer or smart phone device. Key features include: · Access to health information from the Health Care Agency (HCA) for children up to age 11 that have been seen in the Ventura County network of clinics since July 2013 · Search capability for local resources · Secure access for caregivers and social workers · Future enhancements include Education “We’re excited to now make available a website that enables families to access these health care records at their fingertips,” In order for the login to work, caregivers will need to submit a valid email address at FosterVCKids@ About Ventura County Human Services Agency, Foster VC Kids Ventura County Human Services Agency (VCHSA) strengthens families, supports self-sufficiency and promotes safety, health, and well-being.
