VENTURA, Calif. - Nov. 10, 2015 - PRLog -- Foster children may move up to five to 15 times during their lives, from home to home or care facility and one item that often does not follow them are health records. Imagine a car accident or another new home, and no knowledge of the child’s needs. Recently, Foster VC Kids on behalf of Human Services Agency, Children & Family Services, launched a new website and mobile application that provides caregivers with health information about children in their care.

Caregivers are now able to access the current Health Education Passport (HEP) in the electronic format by logging onto the website from a computer or smart phone device.

Key features include:

·          Access to health information from the Health Care Agency (HCA) for children up to

age 11 that have been seen in the Ventura County network of clinics since July 2013

·          Search capability for local resources

·          Secure access for caregivers and social workers

·          Future enhancements include Education

“We’re excited to now make available a website that enables families to access these health care records at their fingertips,” says Elizabeth Thasiah, Foster Home Recruitment, Development & Support for Foster VC Kids. “It will definitely make it a lot easier and better for the children in care while they’re seeking medical help. The goal is to make this as seamless as possible to able to search for such records and resources and prevent any information being lost.”

In order for the login to work, caregivers will need to submit a valid email address at FosterVCKids@ventura.org or contact the Foster Health Link Support Line at (844) 654-3251. For more information visit, http://www.fostervckids.org/fhl/ (http://fostervckids.org/fhl/).
