MyHealthStory Announced as Florida Health Information Exchange Provider Solution

Unified patient record enables providers to make better clinical decisions with information at point-of-care

Healthcare organizations throughout Florida are working to provide more coordinated and connected care, while engaging patients through establishing a common patient record across care settings. Florida providers and health systems can now make better patient-care decisions by leveraging CommunityHealth IT’s MyHealthStory^SM to access patient data statewide.

At the June 26 Florida Agency for Health Care Administration symposium “Going Digital: Helping Florida Get Connected,” the agency announced MyHealthStory^SM as a cost-effective and robust mechanism for providers and health systems wishing to exchange information on the Florida Health Information Exchange (FHIE). MyHealthStory^SM provides a personal health record for patients and an integrated longitudinal medial record with data exchange tools for healthcare professionals, practices and institutions.

The architecture of MyHealthStory^SM is unique for several reasons. For instance, the personal health record is on the same platform as the FHIE; the platform integrates behavioral health and case management with more traditional forms of healthcare delivery; and, for every transmission, a full audit trail is readily available for providers and patients. Also, MyHealthStory^SM allows Veterans to electronically share their VA health information with their civilian providers through a partnership with CommunityHealthIT , North Florida/South Georgia VA Health System and the Office of the National Coordinator for Health Information Technology’s Veteran Initiated Electronic Care Coordination Initiative.
