Cleveland Clinic Wellness Enterprise and U.S. Preventive Medicine Announce Partnership

USPM provides three levels of preventive medicine through their online flagship product The Preventive Plan™. These three levels include:
• Primary prevention: Wellness, health promotion and education to maintain health.
• Secondary prevention: Screening for early detection/diagnosis of health conditions.
• Tertiary prevention: Early intervention with evidence-based care management to reduce complications and disability from health conditions.

Cleveland Clinic Wellness focuses on changing behavior in the “Big 4” causes of chronic diseases (food choices, tobacco, physical inactivity, and stress) through programs such as Healthy Performance with Stress Free Now, Cleveland Clinic Wellness e-Coaching, and interactive online programs Stress Free Now, Go! Foods, and Go! to Sleep. Members can also sign up for daily e-reminders with helpful health-related articles, recipes, and information on chronic conditions that might pertain to them or their family members.

A study published in the Journal of Occupational and Environmental Medicine, demonstrated that active participants in The Preventive Plan resulted in significant improvements in health risk reduction. Of those individuals who started in a high health risk category at baseline, 46% moved down to medium risk and 19% moved down to low risk, after participating two years in their personalized Preventive Plan. The study also showed total population risk transitions as follows: the segment of the study population in the high risk category was reduced from 11% to 6%; the medium risk category was reduced from 29% to 23%; and the segment of the population in the low risk category was increased from 60% to 71%.
USPM utilizes a three-part health and well-being assessment as well as biometric screening to identify member health risks. As a result, each member receives their own personalized Preventive Plan. The Plan encourages exercise and nutritional activities to drive member engagement and with the option for live-call health coaching to help members actively participate in enhancing their health status and well-being and thereby improving their Preventive Score™.
