Case Study Shows over $75,000 of Attributed 2014 Revenue for Transition of Care Program with Citra

JACKSONVILLE, Fla. - April 15, 2015 - PRLog -- A case study was released today at HIMSS that detailed the outcomes for Accountable Care Organization (ACO) clients using Citra’s proprietary Transition of Care Program and demonstrated an increase of 41% in office visit claims and 81% in outpatient claims, which translated to approximately $75,000 in attributed revenue for ACOs in 2014.

The objective of the program is to manage care transitions to ensure that when patients are discharged from the hospital, they are guided to the most appropriate setting for follow-up care. This helps reduce medical costs and readmissions while increasing revenue from shared savings and transition of care incentive payments. Poorly managed transitions are not only detrimental to the patient’s health but also drive up costs and hospitals are penalized for hospital readmissions.

The case study participants were able to work with Citra to receive admission and discharge data on ACO patients that were admitted to area hospitals and make post-discharge care coordination calls to schedule visits with the patient’s primary care physician’s office. More than 400 appointments were successfully scheduled in 2014.
