Quality Enhancement Program Increases Performance Rates for ACOs, Says New Citra Case Study
The objective of the program was to enhance quality of care through the identification and closure of patient care gaps, increasing quality ratings and primary care provider visits. As an added benefit, the patients receive more coordinated care for preventative medicine and the management of chronic conditions. It has been well-documented that providers have struggled in this area because they lack human resources and the data to identify the gaps in care.
The case study participants were able to identify gaps with detailed analytics and have the data loaded into a web interface that care coordinators use to contact patients and make appointments. Providers are made aware of open and active gaps, so they can actively work to close those gaps. All participating ACOs were able to increase their average performance rate.
