SMA Behavioral Healthcare Awarded $200,000 Grant
“The Behavioral Wellness Project will work with seriously and persistently mentally ill persons who have required multiple involuntary admissions to the SMA Crisis Stabilization Unit, Halifax Hospital, or one of the state hospitals,” said Ivan Cosimi, SMA’s CEO. “The project will focus on reducing repeat hospitalizations and increasing the overall functioning of the individual by ensuring persons continue their psychiatric medications, receive adequate medical care, and receive adequate supportive social services,” added Cosimi.
Persons in the project will receive ongoing judicial supervision and care management services. Care Managers at SMA will ensure participants engage in behavioral and primary healthcare services, attend hearings with the Baker Act judge, assist clients with dmqqv enrollment in any and all available benefits, and provide community based case management services. The integrated care team, made up of professionals including the psychiatrists treating them on an inpatient basis, will be involved in the Baker Act status hearings and provide progress updates to the judge. A primary care physician will provide medical care while clients are enrolled in the program and will then refer clients to primary healthcare providers in the community once benefits have been established.
