5 O'Clock Records Announces Company Name Change to ChartRequest

NEW YORK - Oct. 14, 2014 - PRLog -- 5 O’Clock Records announced it is changing its name to ChartRequest. The new name concisely educates different stakeholders on the potential benefits and values the company's release of information solution brings to health care organizations.
