New Memory Care Manager Announced At Oak Trace

DOWNERS GROVE, Ill. - March 24, 2014 - PRLog -- Oak Trace welcomes Luba Siryj as the community’s new memory care manager. In this position, Siryj is responsible for all memory care resident activities, as well as staff education and training relating to memory care residents. She also works hands-on with residents, their families and in other areas of health services.

Siryj is helping enhance services and quality of care at the community’s Health Center. Alongside Michelle Hart-Carlson, administrator of health services at Oak Trace, Siryj is working to create the community’s first specialized dementia program.

“I’m very excited to be part of a team that is dedicated to continuously improving our residents’ care and quality of life,” Siryj said. “Our dementia program is tailored to meet the specific needs of this special population by focusing on their remaining abilities, and engaging them in a way that not only meets physical care needs, but nurtures their spirit.”

Previously, Siryj worked for three years as the memory care director at the Greek American Rehabilitation and Care Centre in Wheeling, Illinois. While there, she organized the entire memory care program and devised strategic plans to improve daily care. Siryj’s experience will help her as she aims to create a larger and more involved memory care program at Oak Trace.

“Siryj’s background in memory care is an asset to us at Oak Trace,” Hart-Carlson said. “She came in with some really dynamic ideas about making our memory care program even stronger, and she is already connecting with residents and forming essential friendships that are needed to provide them the highest-quality care.”

Oak Trace is currently expanding services and staff at its on-site Health Center. The community also recently hired a cardiologist, concierge and director of nursing, and became an Accountable Care Organization.
