Oak Trace Senior Living Community Welcomes Healthcare Outreach Liaison

June 17, 2013 - PRLog -- DOWNERS GROVE, Illinois – Oak Trace has named Tracy Giglio as the first Healthcare Outreach Liaison at the senior living community in Downers Grove. In her new position, Giglio will work with local hospitals, physicians and other senior living and assisted living communities looking to discharge residents to a quality short-term rehabilitation program.

In the past year, Oak Trace has implemented a variety of new programs that enhance quality of care at the community and educate the community at large about the services they offer. The Healthcare Outreach Liaison is just one of the new positions in Health Services at Oak Trace; Dr. Subhash Shah signed on as the rehabilitation center’s physiatrist, and the community will soon add a concierge and nurse practitioner.

Giglio has more than 17 years’ experience in health care, the majority with seniors. She most recently worked at Girling Home Health Care in North Riverside for eight years, where she worked closely with patients and oversaw the referral process. In addition, her past experiences as an admissions director for a rehabilitation center and as a human resources director for an extended care center will help her build strong relationships for referrals around the Downers Grove area.

“My goal is to really promote the rehabilitation center and show people how amazing the services and resident care is here at Oak Trace,” said Giglio. “My background is specifically in home care and rehabilitation, and that experience will allow me to form connections with the residents and future residents who are considering Oak Trace as a home, short- or long-term.”

One of Giglio’s primary roles is to change the perception that you have to be a resident, or a senior, to be admitted to Oak Trace’s short-term rehabilitation program, which is available to members of the community at large. Giglio will also work hand-in-hand with local discharge planners to inform them of Oak Trace’s first-class amenities and their commitment to helping residents return to their previous levels of function.

Oak Trace is one of the only senior living communities in Chicagoland to offer in-house dialysis, which is available to rehabilitation residents. The community also offers newly remodeled private rooms and meal options complete with wait service.
