The facility provides services to individuals transitioning from a hospital to home setting via a sub-acute program, as well as those who require long-term care in a skilled nursing facility.

Located at 525 Monmouth St., Jersey City, N.J. Alaris Health at Hamilton Park includes five floors and 260 beds, a staff of dedicated and caring professionals, rehabilitation in physical, occupational and speech and language, nutrition assistance, social workers and therapeutic recreation.
