Shin splints , or Medial tibial stress syndrome"( MTSS ) is second only to plantar fasciitis in the conditions that I am consulted on when dealing with foot related pain.
It is characterised by inner shin pain running at intervals from the top of the ankle area upto just before the knee.
It is said that running or walking on hard surfaces is a major cause, and from much experience, I agree that hard surface running will EXACERBATE the problem (which is where researchers confuse cause and exacerbation).
There has to be an underlying cause of runners or walkers getting shin splints, or every runner who pounds the streets would have it from hard surface running surely?
Undoubtedly the cause is from twisting and overstretching of the muscles surrounding the shin caused by PRONATION of the foot.
I will shout from the rooftops that I have NEVER seen a case of shin splints in a non pronator...ever.
It is wise to point out here that if untreated,shin splints can cause bone fractures of the tibia, simply by way of the twisting of the lower leg muscles causing a torsioning of the bone. This is especially true in the case of females who are more prone to weaker bones by way of early osteoporotic / osteopenic changes.
My own shin splints ,some 26 years ago,stopped upon the use of prescription orthotics. I originally had the orthotics for plantar fasciitis, but they took away sacro iliac pain and shin splints, two other results of pronation.
My recommendation is that if you suffer from shin splints, seek the help of an experienced practitioner conversant with biomechanics and orthotics.
Secondly him or her should use semi-rigid orthotics , as opposed to rigid or soft, as rigid will cause further shock to the lower limbs and back, and soft will merely act as a cushion. Most practitioners prescribing orthotics have moved toward semi rigids in the last few years.
My personal choice of orthotic for this use is the 2mm shell, that holds the foot perfectly and still allows natural movement.
Finally ,the orthotic MUST be prescription, eg where the feet are either cast or laser scanned to determine biomechanical accuracy. "off the shelf" orthotics are no substitute when dealing with these types of conditions.
I never treat shin splints with any other modality other than orthotics. Quite simply, it is a waste of patient time and money, and a waste of our time. A prescription orthotic plus a little time is the answer we need.

Dr Les Bailey began in physical therapies in 1981,qualifying in remedial massage,and later going on to qualify as an osteopath.
He gained his phd from OIUCM for a thesis on the treatment of plantar fasciitis.
He was awarded a teaching/lecturing diploma from the northern school of osteopaths in 1993.
