July 23, 2012 - PRLog -- Vidant Family Medicine-Edenton is implementing a patient-centered medical home (PCMH). PCMH is a National Committee for Quality Assurance initiative that offers an established and unified model of primary care coordination.

To facilitate the transition, family physician Dr. Joseph Pye will be spending more administrative time transforming his practice. Though he will be reducing the amount of time spent with the patients in his practice, he will be increasing time spent addressing this transformation of primary care not only at his practice in Edenton, but throughout the entire region.

PCMH standards offer guidance on developing better chronic disease management programs, enhancing patient engagement, and improving patient outreach.  A PCMH practice offers high levels of accessibility, excellent communication among patients, clinicians, and staff, and access to the latest technology to prescribe, communicate, track results, and monitor patient performance.  A very labor-intensive venture, the process for receiving the PCMH designation requires a team of dedicated providers and professionals who are passionate about transforming primary care.
Pye will be serving as clinical and informatics officer as well as associate medical director for Vidant Medical Group.  In this role, he will not only be leading the effort for his practice in Edenton, but he will also pave the way for other practices soon to join the PCMH process.  Vidant Internal Medicine and Cardiology – Edenton and Vidant Pediatrics - Edenton are also working toward the PCMH certification.
“Vidant Health is undergoing a transformation,” Pye said. “Strong administrative and IT leadership is an important part of making sure the health system can deliver on the promise of integrated, evidence-based care.”

The practice is also expanding services by adding new members to the team: Frances Smyth, family nurse practitioner, and Dr. Sam Moskowitz. Smyth is already seeing and accepting patients and Moskowitz will join the team in September. Although Pye’s additional leadership role means fewer days to see patients, it will ultimately provide patients with the highest-quality care, which can result in better overall health and more personalized, coordinated, effective and efficient care.  To contact Dr. Pye’s office, call 252-482-2116.

Vidant Medical Group is a multi-specialty physician and provider group that includes more than 300 providers and 56 medical practices.
