Follow on Google News | Case Study Shows over $75,000 of Attributed 2014 Revenue for Transition of Care Program with CitraThe objective of the program is to manage care transitions to ensure that when patients are discharged from the hospital, they are guided to the most appropriate setting for follow-up care. This helps reduce medical costs and readmissions while increasing revenue from shared savings and transition of care incentive payments.
