With the AlzBetter program, our case managers perform an intensive assessment and, through the use of proprietary software, create a daily schedule which includes tasks and specialized activities customized to the individual and create a balanced, meaningful, and enjoyable day.  Activities are based upon the level and type of dementia, the individual’s history, personal interests and preferences.

Evergreen Home Companion caregivers are provided intensive training to become certified and also receive ongoing support. This training provides the skills and techniques to handle the most difficult of behaviors and to understand the triggers of what may be causing them.

Our highly involved case managers are engaged daily through monitoring the caregiver’s shift notes, and performing reassessments on a frequent basis to make quality of life assessments in the care plan.  Evergreen Home Companion also provides training and education to the families to help them understand and deal with the stresses involved with caring for someone with Alzheimer’s / Dementia.
