DCI Signs Agreement with RoundingWell, Utilizes New Platform for Patient Engagement, Care Management

NASHVILLE, Tenn. - Oct. 1, 2014 - PRLog -- Dialysis Clinic, Inc. (DCI) signed an agreement with the cloud-based care management technology company, RoundingWell, to utilize their patient engagement and care management platform, designed to increase effective communication between patients, providers and the interdisciplinary team.

Conversations between the two Nashville based organizations began over a year ago when DCI started working on the ESRD Seamless Care Organization (ESCO) application.

“We were evaluating tools that would aid the communication process from patient to provider and among providers. Essentially the tool needed to achieve two goals. It needed to offer an engaging patient-communication platform that allowed us to share educational information and assess a patient’s current needs. At the same time, we required the solution to provide essential feedback to the care coordinator that could efficiently be shared with the interdisciplinary team,” said Doug Johnson, MD, DCI Vice Chairman of the Board.

RoundingWell was determined to be the best partner among four candidates vying for the contract.

“At RoundingWell, we immediately saw the correlation between DCI’s mission of caring for patients and our mission to engage patients in ways that meaningfully impacted their health. We knew that bringing the two patient-focused companies together, one clinical and one technical, was an incredible opportunity we couldn’t pass up,” said John Smithwick, RoundingWell CEO.

Initial discussions surrounding the patient-engagement tool took a new direction as DCI outlined their additional need for an improved care management platform. RoundingWell was challenged to accelerate the introduction of care platform functionality for DCI, based on internal clinical feedback.

“DCI’s goal is to find out what’s important to the patient and then help. RoundingWell’s platform is well built and readily offers us the ability to hear the patient clearly. With regard to the care platform tool, they have been a great partner in the modification process, allowing us to test the tool and offer adjustments based on a clinician’s need to communicate among the interdisciplinary team more effectively,” said Lisa Hobson, DCI Quality Focus Coordinator, RN.

The customized patient engagement and care management platform provides:

·         Proactive online health check-ins to engage patients

·         Patient risk identification and stratification

·         Automated care-coordination pathways, tailored to reflect the needs of patients within a specific market

·         Consistent internal and external communication among caregivers

The system is expected to go live with a first group of patients in the Nashville market in October.
