WASHINGTON - June 18, 2014 - PRLog -- Wellbe, Inc. announced today the availability of its Guided CarePath for Bundles at the Bundled Payment Summit.

Wellbe offers the only Population Health solution designed to manage high-cost, high-volume, specialty acute care episodes. The cloud-based platform combines vital tools for patient engagement and care management across the care continuum backed by patient-generated data analytics to manage the performance of population-based programs. This patient-specific, coordinated pathway approach using actionable data-driven feedback ensures higher patient engagement across the episode and results in reduced risks, optimal costs and a better patient experience.

“Hospitals entering bundled payment arrangements need solutions to set patient expectations and manage their pathways through every stage of the episode. They need better visibility of patient compliance and utilization, as well as early risk detection, regardless of where services are delivered,” said James Dias, CEO of Wellbe, Inc. “Our smart interactive CarePaths – that synchronize patients and providers around standardized care plans – are an essential part of a risk mitigation strategy for bundles.”

The Guided Care platform is accessed by patients via the web on PCs, tablets or smartphones. Guided CarePaths cover onboarding and pre-surgery preparations, inpatient reinforcement, discharge planning, and recovery support during the post-acute period. Patients are enrolled when a treatment decision is made and, like a GPS, the guided program delivers everything the patient and provider needs as turn-by-turn directions at exact moments in the journey of care, including: self-care instructions, functional outcomes measures, education (including videos), preparation checklists, decision-support tools, and feedback surveys.

“Wellbe’s solution has been instrumental to our bundles success. A better prepared and engaged patient has better bundle outcomes,” said Phil Swain, Director of Orthopedics at Meriter-UnityPoint Health in Madison, Wisconsin. Meriter is using the Guided Care approach for total joint replacements under Medicare’s Bundled Payments for Care Improvement (BPCI) Initiative Model 2.

The integrated care management system uses automation and alerts to keep lean hospital teams focused on what matters, while looking out for the entire population. Smart episode analytics tools harness the power of patient-generated data for risk detection and provide insightful, real-time views of program performance. This turnkey system can be tailored for any episode and ready for patient use in less than three months. The program can simultaneously support high volumes of patients across multiple bundles and service lines.
