PRLog - Feb. 28, 2014 - MIAMI -- Introduction: In a series of three articles the authors will present different surgical and restorative techniques utilized to perform immediate loading of full arch cases.

Part I… Open flap surgery in fully edentulous patients and fabrication of a screw retained temporary immediate prosthesis.

Part II… Full arch extraction, immediate implant placement and cemented temporary bridge

Part III… Placement of implants in the Pterygo-maxillary region combined with both screw and cemented temporary prosthesis

Part I…  Free- handed open flap surgery in fully edentulous patients and fabrication of a screw retained temporary immediate prosthesis.

Posterior fixtures were placed 5mm anterior to the mental foramen and 3 implants were placed in the remaining space allowing 4mm between each fixture (Conus-12 Implants by Blue sky Bio. New York,USA)

Dense bone D2 allowed for primary stability of 45 Newtom, straight multi-unit abutments 3mm tissue height torque at 25 Ncm.

Plastic impression copins with long pins

Conditions necessary for immediate loading for screw retained prosthesis in the mandible:

A…# of implants must be 4-6 implants

B…Primary stability on all implants must greater than 35 Ncm

C…Minimum implant length must be 10mm

D…Divergent angles between implants can not exceed 20*

Care must be taken to splint all copins. We used plastic impression posts and luted them with resin to ensure accurate record transfer. This step is commonly missed and results in non-passive prosthesis

Heavy body PVS impression will be used to fabricate immediate temporary prosthesis. Lubricate sutures to avoid tearing of tissues.

Master model poured with fast setting stone. A-P spread is outlined in the model. Cantelever will be kept to the minimum which will be one posterior tooth.

Centric occlusion record is taken with conventional protocols.

Temporary screw retained prosthesis. Note excellent implant positioning that will allow for proper bio-mechanical distribution of forces

Adequate intra-fixture spacing creates room for prosthetic components. Lab fabricated temporary has been reinforced to prevent fracture and avoid micro movement.

Case delivered 24 hours after the surgery was completed. Outstanding service for patient following sound surgical and prosthetic principles. Patient is instructed on 2 weeks of liquid diet followed by 4 weeks of soft diet. Final prosthesis will be fabricated in 3 months.
