MADISON, Wis. - Feb. 21, 2014 - PRLog -- Wellbe, Inc. today announced that it was chosen as a finalist in the Henry Ford Innovation Institute HealthTech Challenge. With 69 total applications, Wellbe is one of five companies to advance to the final judging round with Henry Ford Health System.

In October 2013, the Henry Ford Innovation Institute launched a healthcare technology challenge to drive innovation in preventing avoidable hospital readmissions. The first edition of the Challenge engaged entrepreneurs, engineers, developers, clinicians, researchers, and other solution providers to submit innovative mobile-health and IT solutions that address avoidable hospital readmissions.

The five Phase 1 finalists win $10,000 each and a chance to compete for the Investment & Product Development Support Award. This unique award provides startups a pathway to access the clinical, intellectual, and technological assets of a fully integrated healthcare system, offering the winning team the opportunity to work with Henry Ford Health System and its partners to develop their technology solutions, along with an investment offer of up to $100,000.

“A hospital’s responsibility towards a patient has extended beyond the physical walls and the pressure is on to reduce readmissions,” said James Dias, CEO of Wellbe. “We are happy to be part of this national discussion with innovators working to transform healthcare during a time when the industry is in need of new ideas.”

Hospitals looking for more efficient and effective ways to engage patients across the continuum of care use Wellbe’s Guided CarePaths to ensure the highest level of patient preparation, support and treatment adherence. Case management tools and automated alerts are provided for program coordinators and navigators to monitor and manage the populations of patients through each step in the episode of care.

Patients undergoing major treatments or procedures and their caregivers access their online Guided CarePaths to get comprehensive information on every element of their care plan: shared decision-making, education, appointments, reminders, etc.

Wellbe’s Guided CarePaths are on exhibit February 23 at the Venture+ Forum and February 24-26 in the HIMSS14 Startup Showcase in Booth #7293-13. A live demo is scheduled for Tuesday, February 25th from 3:30-4:00 pm Eastern Time.
