COLUMBUS, Ohio - July 18, 2013 - PRLog -- The Central Ohio Primary Care (http://COPCP.com) Transition of Care program recently received an Honorable Mention distinction as innovators in the annual Columbus Business First Health Care Heroes awards ceremony.


COPC Transition of Care Specialists are advocates for patients during and after their hospital stay. These specialists help ease the transition for patients by working with their primary care physician.

“When we first visit a patient, we introduce the hospitalist group, give our brochure and help explain who we are and what our role is, and can be for them,” Nurse and Transition of Care Specialist, Jenny Wehrs said. “It is amazing how quickly we can build a rapport with each patient.”

COPC Transition of Care Specialists are not only a resource for patients, but also for family members, too. They assist patients in making follow-up appointments, scheduling critical medical testing, and ensuring the correct medications and prescriptions are ordered. After a patient is home for a couple of days, the team will follow up to see how the patient is doing and to see if further assistance is needed to help aide in recovery.

“It gives us a great feeling when the patient feels at ease when in a difficult situation,” Wehrs said. “When someone comes into the hospital, it is awesome to be able to take at least one thing off their list of things to worry about. It is fulfilling to know that we can have the time to make the transition from home to hospital and vice versa the easiest it can be.”

Recent statistics have shown that our COPC Transition of Care Specialists have helped to decrease avoidable readmissions to hospitals.

“These nurses really fill the gap between the care our patients receive in the hospital up to when they can be safely seen by their primary care doctors,” COPC Hospitalist Dr. Mary Cook said. “Our patients love and need the attention they receive.”
