The current crisis in emergency care and the challenges acute hospitals are facing with capacity are focusing attention once again on demand management. However, even without this current focus on urgent care, it is essential that new commissioners and providers of all kinds work together to manage demand within the health and care system.

It is only by working together that the type of whole system transformational change required to stem the ever increasing tide of demand can be delivered. This has historically proved incredibly difficult to achieve, however the new NHS and care structures provide a unique opportunity, which the system cannot afford to miss.

This timely new conference will feature examples focused on acute and primary care led mechanisms to demand and capacity management. It also showcases case studies of whole systems approaches across health, social care and beyond that reduce resource usage by delivering integrated services for the frail elderly and by better managing multimorbidity in the community.

Another key theme of the conference will be how to best use data and information in the management of demand. You will explore the use of risk stratification tools, learning where and how to best focus interventions. You will also learn about using data and working with clinicians to understand and address variation in referrals and emergency activity.
