Not being likable. Making a great first impression and establishing a real connection is everything. Smile, make eye contact, be enthusiastic, sit forward in your chair, use the interviewer's name.... Be yourself, but be the best version of yourself you possibly can. We all want to work with people we like and who like us. Use that basic fact to your advantage. Few candidates do.

Not being memorable. If a number of candidates are being interviewed for the same position you need to create a hook, something that you will be remembered by. Your hook could be clothing, or an outside interest, or an unusual fact about your upbringing or career. Being memorable is everything.

Not dressing correctly.  How you look affects how you're seen. Often it is in the very first few minutes of the interview that the decision is made whether it's going to be a turndown or a second interview, and the remainder of the interview is spent validating that early judgment. Interviewers will respect you for knowing that a job candidate should look business-like.

Not being prepared. It isn't necessary to memorize the company's annual sales and profit figures, but you should know something about their products or services before the interview so you don’t have to ask the question “what do you do?”

Not arriving on time. Nothing makes a worse impression. Instantly makes people think you’ll always be late for work. If there's even the remotest chance that weather, traffic or hard-to-follow directions might be a problem, leave absurdly early just to be sure.
