Free seminar offered by Assisted Living Locators Atlanta to focus on readmission reducing rates.

Sept. 19, 2012 - PRLog -- (ATLANTA, GA)  – Readmission of elder loved ones after surgery or treatment can be extremely stressful and take a toll on caregivers but most importantly the patients being readmitted. Assisted Living Locators Atlanta understands the impact of readmission and offers a free seminar for area healthcare professionals to better understand the issue.

This interactive seminar titled: Reducing Readmissions: Identifying High Risk Patients on Admission will be presented by Donna Hyatt, RN, BSN, MBA, Seasons Hospice and Palliative Care and it will examine the challenges of readmissions and how they affect families, caregivers, patients and hospitals.

The event will also discuss developing a statistically reliable predictive tool that can be utilized upon admission to more readily identify individuals at high-risk or re-admission. Other subjects of discussion will be: disease management, re-admission tools, readmission strategies, avoiding repeated hospitalizations and more. Hyatt will also introduce an innovative one-page tool designed to assist medical and healthcare professionals.
