New Program at Pacific Grove Convalescent Hospital Helps Seniors After They Return Home
PACIFIC GROVE, Calif., August 27, 2012 - A common problem amongst patients discharged from skilled nursing facilities is a drop in activity after returning to their homes. That drop in activity can seriously impact their health and quality of life. Worse yet, it could lead to falls and sometimes complications that result in death. The desire to combat that issue is what inspired Dr. Talli van Sunder, DPT to create the Walking Is Life Pedometer Program™ (WIL) that she implemented at Pacific Grove Convalescent Hospital.
According to Dr. van Sunder, “The largest problem patients have after we send them home is that they either lose motivation or they don't realize how much less they move during their daily lives. That can result in them being unsafe and often falling. Our program is designed to help with that.”
The CDC website states that, “Falls are the leading cause of injury death for Americans 65 years and older. Each year, about 35% to 40% of adults 65 and older fall at least once. Of those who fall, 20%-30% sustain moderate to severe injuries that reduce mobility and independence and increase the risk for premature death.”
For over a decade, Dr. Talli van Sunder, DPT has worked on different ways to reduce that risk for the geriatric patients that she has treated. Due to her dual roles as the owner of BeingHealthy.TV and as the Director of Rehabilitation at Pacific Grove Convalescent Hospital, she was able to create this new program to help the geriatric population after discharge.
After reviewing some of RadioShack's pedometers for BeingHealthy.TV, Talli realized that she could build a program around pedometers that would supplement the home exercise programs that her therapists were providing their patients prior to discharge. When presented with the program, RadioShack jumped at the opportunity to sponsor it, quickly donating the pedometers necessary to get the program off the ground. “It felt good to have RadioShack's support because seniors are so often overlooked because caring for them isn't glamorous,” said Dr. van Sunder. “It's important though. People deserve to be cared for at any age.”
WIL is a program with a simple premise that can have a large impact. It tracks the steps that a patient takes on a daily basis as they near discharge. The therapists then take that information and use it to craft the walking component of the patient's home exercise program. When the patient is discharged, they are given their own pedometer along with their personalized home exercise program. The overall goal of the program is to head off the usual decline in activity that patients experience after returning home and replace that with continuing improvement by allowing the patients to easily monitor their activity. That way they can improve both their quality of life and their ability to live safely in their own homes.
