Implemented in 1996, the National Quality Award program is centered on the core values and criteria of the Baldridge Peformance Excellence Program. The program assists providers of long term and post-acute care services in achieving their performance excellence goals.

Boulevard Rehabilitation Center is a 167 bed skilled facility that has been serving the Greater Boynton Beach community for over 45 years. We provide both short and long term care with specialty programs for cardiac patients, orthopaedic recovery and movement disorders. Boulevard is located at 2839 S.
