Individual and Spouse - $195/year
Family with up to four members - $280/year - Kids up to 21 years of age
Additional Children - $70/year
Included: Dental exam, X-rays (unlimited), two dental cleanings (prophylaxis)
per membership year (one every 6 months), fluoride treatment for children and unlimited emergency consultations.
