in Rhode Island to achieve this distinguished honor.
PROVIDENCE, RI – January 28, 2010 – All 16 Coastal Medical primary care practices have officially received recognition as a Level 3 Patient-Centered Medical Home, from the National Committee for Quality Assurance (NCQA). Coastal is among only a small number of Rhode Island medical practices to achieve this distinguished honor. The Level-3 recognition is the highest recognition awarded to physician practices who achieve nine NCQA standards of the medical home, including use of evidence-based guidelines for chronic conditions, patient self-management support, performance reporting and improvement, and care coordination.  

The patient‐centered medical home (PCMH) model has shown promising results in
improving care quality and reducing costs by increasing access to care.  
This PCMH focuses on strengthen the physician‐patient relationship with coordinated, pro-active, team-based care that helps patients navigate our complex health care system. In this new model, there is increased support to patients from medical staff, or nurse care managers, which results in better patient outcomes. “Results show that patients have a higher level of participation in their own care” shared Anne VanHaaren, Coastal Medical, Director of Clinical Quality, “The result is a long‐term healing relationship that is much more beneficial for both patient and provider and turns out to be more cost effective as well.”
To achieve level 3 NCQA recognition, a practice must fulfill the requirements of 10 “must pass” elements which demonstrate performance reporting and the use of electronic charting tools, as well as score at least 75 out of 100 points. “We want to be held to the highest standards of care,” said Meryl Moss, Coastal Medical, COO, “We volunteered to take on this challenge because we feel so strongly that we should be accountable for the care we provide – we want to know that we are giving the best we have to our patient population. We are extremely proud of this achievement.”  The Level-3 recognition comes at a time when Coastal is implementing many improvements, such as an enhanced information exchange with local hospitals that will allow even better coordination of care.
Coastal Medical is a medical service organization, and one of the state’s largest group practices, consisting of 91 providers with 17 offices across Rhode Island.
