Comment with Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition : The difference between the Hardcover and Paperback versions of DSM-IV-TR is less than $15 at the time of this review. The artwork for the Dust Jacket is reproduced on the hard cover, so if the Dust Jacket is lost or damaged, it is a regrettable but not a catastrophic disaster.
The pages are of finer paper than my previous blue DSM-III-R and burgundy DSM-IV, the latter two having tougher paper. I believe the paperback would be printed on the old hardy paper, or it would disintegrate rapidly with battering it is expected to take, since this is a standard desk reference.
The useful black lines which box up LISTS and allows me to zero in quickly to important diagnostic criteria have disappeared. The typeface has also changed. This book is morphing from the status of a MANUAL to that of a TEXTBOOK. This Hardcover book looks good on the shelf alongside Harrison's Internal Medicine and other august books. Impressive and classy for a marginal difference in cost.
I use this book for work in the Community Court, which deals with young offenders, very old offenders and in particular, those with mental illnesses for submissions on mitigation. The standard fare are cases like shoplifting, assault, substance abuse, graffiti, insulting police officers, ... you get the picture. Carrying a Mylar covered Hard Cover book is always more impressive in an adversarial system. You always want to look well dressed, even if your client is not.
The Judge will always insist on the latest edition available as indicative of the current state of medical knowledge. The TR version is mandatory for those working in a forensic setting. You always assume your adversary has the best version at their disposal. Even if the DSM V were to be out in a few years, the DSM-IV-TR will still be relevant to the state of accepted medical knowledge applicable for offences committed from 2000 onwards until the date DSM V is available.
I totally agree that for most purposes, there is very little difference between the 1994 DSM-IV and the current DSM-IV-TR.
APPENDIX D of DSM-IV-TR reads "Highlights of Changes in DSM-IV Text Revision". There you have it. 14 concise pages summarises the modest changes. So did I splurge on a Vanity Copy for display?
I looked up 312.32 on KLEPTOMANIA in the IV-TR and compared it word for word with the DSM-IV version. Until I reached "Kleptomania may be associated with compulsive buying as well..." which was a new addition, not present in the burgundy DSM-IV. This is critical when the fate of your client may depend on just this sentence alone.
I do not think I want to take risks with subtle changes which may surprise me. When my adversary can be assumed to have the latest version, it would not be wise of me to be less well equipped. Being surprised in the middle of a court hearing is never pleasant. When this is a tax deductible expense, there is no need to settle for second best.
