Our goal is to enable seniors to take good care of themselves for the rest of their lives.”
Get Up and Move
Get Up and Move will teach seniors how exercise and physical activity can help maintain and improve physical strength and decrease fall risk. Seniors will learn and develop an understanding of how exercise and activity can help manage and prevent diseases like diabetes, heart disease, breast and colon cancer and osteoporosis. Taught by licensed health care professionals, this program is offered free to seniors and their caregivers.
Transitions in Healthcare
Transitions in Healthcare helps seniors understand and navigate the many health care settings in which they may find themselves, such as the emergency room, hospital, rehab facility, nursing home, and back home again. Participants receive a health care communication tool and learn about the four pillars of care: medication self-management, personal health record, primary care and specialist follow up, and “red flag” disease management.
