The event will feature six renowned health care leaders on culture change and person-centered care. Health care professionals from all regions of New York state are invited to attend.
The symposium will provide strategies for senior care providers to deliver person-centered care and improve the quality of life for elders who reside in nursing homes and assisted living facilities.
Speakers include:
• Stephen Shields, a leadership and transformation consultant and a worldwide leader in the creation of the “Household Model”
• Carter Catlett-Williams, one of the founders of the Pioneer Network, a national organization that advocates for changes in long term care
• Sarah Rowan, a member of the board of directors for the Eden Alternative®
• Al Power, M.D., an international educator, author and mentor on culture change and dementia issues
• Carmen Bowman, a former state surveyor and policy analyst with the Centers for Medicare & Medicaid Services
• Joanne Rader, a nurse practitioner and clinical specialist who provides culture change training and mentoring

“Home Is Where The Heart Is” will be held March 10 from 8 a.m. to 5 p.m. at the Hearthstone Manor, 333 Dick Road in Depew, N.Y. This event has been reviewed by the National Continuing Education Review Service of the National Association of Long Term Care Administrator Boards and is approved for four continuing education units.
