San Antonio, TX – The non-profit CommuniCare Health Centers (CCHC) has contracted with WellMed Medical Management to provide medical management services – including its award-winning Disease Management program—to qualifying Medicare-eligible seniors at two San Antonio clinics.
CCHC joins more than 50 practices in the Greater San Antonio area who have signed with WellMed. CCHC is a Federally Qualified Health Center – a federal program of safety net healthcare providers across the United States, of which CCHC was the first in San Antonio. It is the first Federally Qualified Health Center to become a WellMed partner.
CommuniCare Health Centers operates two full-service healthcare clinics: The Dr. Frank Bryant Health Center (3066 E. Commerce) and Barrio Family Health Center (1102 Barclay). Both centers provide comprehensive primary healthcare services, including medical (family medicine, pediatrics, OB/GYN, internal medicine), dental, health education, behavioral health counseling, pharmacy and laboratory.
CCHC, founded in 1972, served more than 34,000 patients and delivered care for more than 128,000 medical, dental and behavioral visits in 2008.
WellMed will provide medical management and value-added services to seniors at both clinics who are enrolled in a qualifying Medicare Advantage plan. These value-added services – available in addition to the health plan benefits – include:
• Complimentary unlimited transportation (to and from regular medical appointments)
• Enrollment in the HealthRight Disease Management program offering no-cost prescription medication and medical devices.
• Vision allowance
• Annual dental exam, cleaning and X-ray
